The Official Affidavit does not have a place to put personal payroll deductions with the other payroll deductions.
You can enter them in the section for Other Deductions, but please be aware that anything you enter in this section will not carry to the child support guideline section, the budget report, where other financial reports. It will be for the Affidavit only.
That better thing to do is simply to enter them as personal expenses, as shown below:
